The ingredients most likely to worsen a damaged skin barrier are fragrance, alcohol denat., and high concentrations of certain exfoliating acids — and the frustrating reality is that all three appear in many otherwise well-formulated moisturizers.
Fragrance is the leading cause of contact sensitisation in leave-on skincare. This applies to both synthetic fragrance (listed as 'fragrance' or 'parfum') and natural fragrance — essential oils like lavender, citrus, eucalyptus, and tea tree are common sensitisers. On a compromised barrier, fragrance molecules penetrate more easily than they would on intact skin, which increases both the risk of sensitisation and the likelihood of an acute inflammatory response. Fragrance-free is not a marketing preference; on damaged skin it is a practical necessity.
Alcohol denat. (denatured alcohol, SD alcohol) evaporates quickly and disrupts lipid organisation in the stratum corneum. It is used in lightweight formulas to improve skin feel and absorption speed, and in small amounts in otherwise-fatty formulas it is often tolerable. But in high concentrations — particularly when it appears in the first five ingredients — it can actively worsen barrier function. Fatty alcohols (cetyl alcohol, cetearyl alcohol, stearyl alcohol) are structurally unrelated and are not a concern; they are emulsifiers and emollients.
High concentrations of AHAs (glycolic acid, lactic acid, mandelic acid) and BHAs (salicylic acid) exfoliate the stratum corneum. That process is useful for some skin goals but actively counterproductive when the goal is barrier reconstruction. Keep acids away from your routine until the barrier has stabilised — typically four to six weeks — then reintroduce them slowly, once or twice a week, with a ceramide product available to buffer irritation.
Essential oils and plant extracts that contain significant linalool, limonene, citronellol, or geraniol — common natural fragrance components and allergens — should be approached with the same caution as synthetic fragrance. 'Natural' does not mean non-irritating; for a sensitised barrier, it frequently means the opposite.
Retinoids warrant a brief mention: they are valuable for long-term skin health but can transiently worsen barrier function during initial use ('retinoid purge' or retinol sensitivity). If you are dealing with acute barrier damage, this is not the moment to start retinoids. Establish barrier stability first, then introduce retinoids at a low concentration with a ceramide buffer.
Physical and mechanical irritants matter too: very hot water strips lipids from the skin surface; long showers and baths compound this effect. Abrasive cleansing cloths, sonic cleansing brushes, and physical scrubs should be suspended during active barrier repair. A gentle, fragrance-free, low-surfactant cleanser applied with fingertips — rinsed with lukewarm rather than hot water — is the right companion to a barrier repair moisturizer.
