When the ratio is roughly equimolar, with ceramides as the dominant class, the bilayers are most structurally stable and most effective at preventing transepidermal water loss.
The practical reason this matters when shopping is that many moisturizers market themselves as ceramide products but omit cholesterol and fatty acids entirely. Ceramides alone can still provide some benefit — they are a meaningful structural ingredient — but without the supporting lipids, the lamellar assembly is incomplete. What you can read from the list is whether all three lipid classes are present and whether ceramides appear before the 1% threshold — typically before common preservatives like phenoxyethanol or sodium benzoate.
Some brands explicitly market their products as formulated to a specific lipid ratio, and a few have published in-house data supporting those claims. When evaluating those claims, look for whether the brand specifies the ceramide types used (ceramide NP, AP, EOP, NS are the most studied) and whether cholesterol and fatty acids are both named in the formula. A brand that makes a 3:1:1 claim but lists ceramides at the bottom of the ingredient panel is using the number as a marketing figure rather than a formulation fact.
For most shoppers, the simpler version of this guideline is: look for ceramides, cholesterol, and at least one fatty acid all present in the same formula, with ceramides appearing before the first preservative.
